Add rate limiting middleware to server.py

Implemented a custom RateLimitMiddleware in server.py to limit requests to 60 per minute per IP address, as configured in src.config.settings.

• Added RateLimitMiddleware class.
• Added app.add_middleware(RateLimitMiddleware) before CORS middleware.
• Added necessary imports (Request, BaseHTTPMiddleware, settings).
• Verified logic with standalone test script (mocking missing dependencies).
• Verified file syntax.

This addresses the security vulnerability at server.py:120.
