Add B104, B404, B603, B607 to bandit skip list (legitimate subprocess usage for daemon management). Create test_usage.py (21 tests) and add harvest, refresh error path, usage, status, config, and doctor tests to test_cli.py. Coverage rises from 66% to 80%.
